Precision or “personalized medicine” is an innovative care model designed to optimize the efficiency and therapeutic benefits for patients by incorporating multiple, patient-specific variables into clinical decision making. Common variables considered are age, comorbidities, and potential drug-drug interactions. Pharmacogenetic (PGx) testing is also becoming a commonly used tool, with FDA and CPIC dosing guidance published for many commonly prescribed medications. However, incorporating PGx with other variables into a clinical workflow while limiting "alert fatigue" is challenging. This presentation will explore resources and tools that allow effective integration of PGx and precision medicine into the residential care setting.
